It clearly mentions if you say YES to 12a or b, then only go thru 12c and d. So leave those blank.

SO WT TO WRITE FOR COUNTRY OF RESIDENCE? NEPAL OR USA.?I HAVE BEEN HERE IN USA SINCE 2013 some says nepal some says usa and still doubt in part 2- 25 b.my visa(I94) expired on aug 2014 but I 20 expire on 2016 which one to write.plzzz let me know

Country of residence is USA. If u r on f1 now, the date of expiration of i-20 is the validity if ur status not the visa. So u gotta put i-20 expiry date.
